Starting physical therapy can feel like a big step — especially if you’ve been dealing with pain or a complex issue for a long time.
At Thrive 4 Life Physical Therapy, we take the time to truly listen, evaluate, and create a plan that is specific to you. Your care is never rushed, and you are always an active participant in the process.
Complete Your Intake Forms
You’ll receive an invitation to securely complete your medical history and consent forms online before your visit.
Gather Important Information (if available)
- Physician referral (if applicable)
- Imaging reports (MRI, X-ray, ultrasound)
- Previous evaluations or lab results

Need More Time?
If you have a complex history or are traveling from out of town, extended sessions (up to 2 hours) are available.
To get the most out of your visit:
- Comfortable clothing (shorts, tank top, or sports bra recommended)
- Athletic shoes or footwear you regularly use
- Orthotics or insoles (if applicable)
- Any relevant medical records
We often assess movement and mechanics, so seeing your normal footwear and how you move is an important part of your evaluation.
During Your Visit
We’ll Take the Time to Understand You
We start with a detailed conversation about your symptoms, history, lifestyle, and goals.
Full-Body Assessment
Your therapist will evaluate how your body moves, including:
- Posture and alignment
- Strength and mobility
- Movement patterns
- Areas contributing to your pain or dysfunction
Pelvic Health (If Applicable)
For pelvic health concerns, your evaluation may include:
- External assessment of hips, back, abdomen, and pelvic floor
- Internal pelvic floor assessment (vaginal and/or rectal), if appropriate
These assessments help evaluate muscle strength, coordination, and tissue health — but are always optional and performed only with your full consent. Your comfort is our priority, and care is always adapted to your needs.
Your Plan of Care
Before you leave your first visit, you’ll have:
- A clear understanding of what’s going on
- A personalized treatment plan
- Hands-on treatment (when appropriate)
- Guidance on next steps and home exercises

Billing & Insurance
We are a cash-based, out-of-network practice, which allows us to provide more personalized, higher-quality care.
- Payment is due at time of service
- We accept credit cards, HSA/FSA, cash, and check
- Superbills are provided for possible reimbursement
